The record proves the visit, the tracker protects the plan

A chiropractic record exists to prove that a given encounter happened and was medically appropriate: the PART findings, the AT modifier decision, the ABN when maintenance care begins. It is documentation of what already occurred. Every entry looks backward, and correctly so.

A care plan tracker looks forward. It knows the patient bought twenty four visits, is at eleven, is trending at one visit a week when the plan said two, and has a re exam due next Thursday. None of that is a documentation fact yet, which is exactly why it needs somewhere else to live.
